Aware Recovery Care (ARC) offers a holistic approach to treatment and changes the way clients receive care throughout the northeast. ARC is a year-long, home based, addiction treatment program that works collaboratively with a multidisciplinary team to assist clients in their first year of recovery.
Position Summary:
The In-Home Withdrawal Management (IHWM) and Medical Assisted Treatment (MAT) Care Coordinator is responsible for the coordination and care management of Aware Recovery Care clients who begin their treatment with IHWM. This individual, as part of a multidisciplinary Addiction Care Team, also manages services to clients in the Medication Assisted Treatment (MAT) program, whether through ARC or Contracted providers, that are deemed necessary or beneficial to the overall progression of the client’s health.
*This is a full time opportunity that pays $65K to $80K annually based on education and experience*

Involvement in all aspects of MAT including Speranza, reSET, reSET-O and Kaden
Order, schedule, and manage Vivitrol injections
Complete PMP checks for ASAMs
Proper documentation of telephone and verbal orders from Clinicians, working knowledge base to carry out those orders as received in the IHWM and MAT Program levels.
Driving to and meeting clients in their home environment.
Assessment and development of an appropriate treatment plan to meet individual ARC client needs with understanding and consideration of client care goals as voiced by the client, making the client a major part of his/her treatment and recovery.
Regular evaluation of client progress, including crisis intervention as appropriate. Follow up after IHWM is completed for ongoing evaluation and study data.
Clinical documentation in an Electronic Health Record (EHR) in real time
Coordination of services with the client, family and all providers involved in the treatment plan.
Maintain up-to-date education on current addiction treatment trends, including knowledge of Internet and local resources for those with substance abuse.
Provide advocacy as needed to ensure the client knows and understands his/her rights.
Supervision and delegation of the services rendered to the client by the CRA in accordance with organization IHAT, IHWM and MAT policies.
Identify client and family needs for services or other community resources and refer for same when appropriate.
Participate in orientation, teaching and supervision of other client care staff.
Involvement in self-evaluation and professional development, IHAT Certification, IHWM and MATS training.
Participate in Case Review, supervision, and continuing education.
Collaborate with IHAT Institute for ongoing data collection for Clinical Studies.
Performs miscellaneous job-related duties as assigned
One year of professional clinical nursing experience in home health care or at least one year of SUD and/or substance use withdrawal experience within the past three years.
Fundamental knowledge and understanding of most current medications prescribed in Detoxification and MAT settings.
Attend scheduled In-service Programs to advance knowledge of MATS and stay current with regulations and evolution of treatment with IHWM and MAT modalities.

Skills & Qualifications
LPN or RN with an active license to practice in the stateOne year of professional clinical nursing experience in home health care or at least one year of SUD and/or substance use withdrawal experience within the past three years.
